When you schedule your appointment with us, you should receive a link to your Patient Portal called myPatientVisit where you can log in and fill out your demographic information, patient intake form, upload your insurance card and driver's license, and sign the HIPPA Acknowledgement and Financial Policy Forms. This will ensure the check in process is smooth and seemless during your first appointment with us.

If you are having difficulty filling out the information online, we would be happy to assist you to fill out all of the relevant paperwork in the office when you arrive. Please make sure to arrive 10-15minutes before your scheduled appointment time so we can assist you.
